Instructions for Starting an Application

On the left hand side is the menu, which will vary depending on your role.  If you are a program director you can access the different areas of your application. Under the first section called "Application" you can start your application or continue an application in progress. Under "Additional Forms" you will find links to enter information about your fellows, faculty, program director and site.

You must fill in all information to complete your application.  You will be unable to submit your application without completing these additional forms under the "Administration" tab. Please note that it is your responsibility to keep these areas up to date at all times, you are welcome to edit these after submitting your application and we encourage you to. If you are changing the program director you can indicate that in the contact form, but please also reach out to EUFAC for an official approval of the change for accreditation purposes.

Once your application is complete a link to pay the accreditation fee will appear in the "Pending Payments" section of your main menu. When your application has been completed it will be sent to the Chair of your department to review and verify. Once it has been verified it will be sent to EUFAC to review.

Completing the Application

There are 7 main sections with 32 sub-sections with approximately 300 questions. We estimate the application will take between 1-3 hours to complete depending on what information you have available. To help you gather information you can download the questions in PDF format to review.

You can complete the application in whatever order you want. Once you initiate the application a menu will appear allowing you to go to any section. Upon completing a section when you click "Next" that data is saved and a green check mark will appear on the menu to show that you have completed it. 

Payment

Once you have submitted your application there is an option to pay your application fee on the left hand menu under "Pending Payments". Click "Pay now" and submit payment using a credit card (all payments are processed securely by PayPal). If you need to pay using an alternate method please contact EUFAC. 

If you need to forward the payment information to an administrator or finance staff member, you can do so by clicking on “Email Payment Link”. This will open a window to allow you to enter the staff member’s email address and send an automated email to the staff member with a link to submit the payment. The staff member may select to pay via credit card or indicate that the payment will be made via check.

Chair Verification

Once an application has been completed it will be forwarded to your chair to review. If you are a department chair who has logged in to verify an application please click on "Applications for Verification" on the left hand menu. From there you will see any pending applications. Click "View Report" next to an application to review. You can review the entire application your program director has submitted. You are required to assess this for accuracy and then  click "Complete Verification". You can accept or deny the application. If you deny the application you can enter comments for your program director who can then fix any mistakes. Once the application is ready and you approve the application, you will need to type in your full name as a digital signature and submit. EUFAC will not review any applications until the department chair has verified them.

Fellows

If you are listed as a fellow on an application you will have a link on the left menu called "Tasks". Click this and you will see if there are any pending questionnaires to fill in. If there is, click the "Edit" link to the right of the listed questionnaire to start it. When you have finished answering all questions make sure the click the "Completed" box and submit. These questionnaires are critical to ensure your fellowship program remains accredited. These questionnaires are not visible to your program director. Faculty

If you are listed as a faculty member on an application you will have a link on the left menu called "Tasks". Click this and you will see if there are any pending questionnaires to fill in. If there is, click the "Edit" link to the right of the listed questionnaire to start it. When you have finished answering all questions make sure the click the "Completed" box and submit. These questionnaires are critical to ensure your fellowship program remains accredited.
